FIG. 4 is a right side view thereof, the left side view being a mirror image;

FIG. 6 is a top plan view thereof.

The bold solid lines represent fold lines in FIGS. 1-6, and the dashed lines represent intermittent cut lines in FIGS. 1-4. The thickness of the blank is that of a conventional paperboard.
